Open records request form

The open records request form is used to request any type of report that the Montgomery County Sheriff’s Office has completed. This form can be used for arrest, accident and incident reports.  Simply click the link below and print out the form. Then bring the completed form to the Montgomery County Sheriff’s Office with valid identification. There is a $5.00 charge for providing a report.
